HIPPA Rules
First name is fine, we just don't want to say "Hey you" (our Mothers raised us better than that)
If you give us an email, we'll be able to answer you more easily. We will NOT use this email for anything else, nor will we share it with anyone else. We won't even play games like sharing it with "other internal organizations" or any of that cleverness. We'll use it to talk with you about your question, nothing more.
This is OPTIONAL. If you prefer to have a phone conversation (instead of giving out your email or just want good old interactive communication), no problem. Let us know the number and a good time to call (use the comment box below), we'll give you a shout. Same rules apply here as above: we won't use your number of anything else nor give/share/send it to anyone else.
